The general philosophy in reporting and documentation process seems to have always been, “Capture the information and we’ll figure out what to do with it later. So long as it’s captured, that’s what matters.” I’d say this approach is akin to “shoot first and ask questions later.” And while it’s true simply capturing the information once was enough, to quote Bob Dylan, “Times, they are a-Changin'”.
